Les rechutes d'accès palustres à Plasmodium vivax apparaissent rarement après deux ans. Nous rapportons ici une rechute vingt ans après un séjour potentiellement infectant. Cette reviviscence est survenue sur un terrain fortement immunocompromis: patient porteur de deux néoplasies au stade terminal, traitées par radio-chimie et corticothérapie, présentant une anémie et une thrombopénie. La répétition des recherches parasitaires a permis la découverte de Plasmodium vivax.
