Nous rapportons le bilan de 10 ans de collaboration avec la Fondation «Terre des hommes» Pays-Bas, concernant le transfert sanitaire aux Pays-Bas de 41 enfants porteurs de pathologies hors de portée médico-chirurgicale à Brazzaville. Ils étaient âgés en moyenne de 3 ans et 6 mois (extrêmes: 2 mois - 15 ans). Il s'est agi de 33 cardiopathies non cyanogènes dominées par les communications inter ventriculaires (CIV) (n=10) et 11 cardiopathies cyanogènes dont la tétralogie de Fallot (n=5). Les autres pathologies étaient: osseuse (n=3), vésicale (n=2), pulmonaire, tumorale, ophtalmique respectivement dans 1 cas. La chirurgie a consisté en une réparation complète dans 19 cas, palliative dans 9 cas. Deux enfants se sont avérés inopérables. Les huit autres pathologies extra-cardiaques ont bénéficié d'une chirurgie spécifique pour chaque cas. La durée moyenne du séjour aux Pays-Bas a été de 1 mois 13 jours (extrêmes: 1 - 12 mois). L'évolution a été favorable pour 35 enfants, toutes pathologies confondues. Quatre décès sont survenus aux Pays-Bas et deux au Congo. L'organisation de transfert de technologie serait probablement dans l'avenir un meilleur choix.
We report the 10 year assessment of collaboration with the Foundation "Terre des hommes" concerning the medical transfer in the Netherlands of 41 children carrying pathologies which couldn't be treated or operated on in Brazzaville. The average age was 3 years and 6 months old (extremes: 2 months - 15 years). 33 non cyanogen cardiopathies dominated by ventricular septal defect (VSD) (n=10) and 11 cyanogen cardiopathies among them the tetralogy of Fallot (n=5) were admitted. The other pathologies were respectively: osseous (n=3), vesical (n=2), pulmonary, tumoral, ophthalmic in 1 case. The surgery consisted in a complete repair in 19 cases, palliative in 9 cases. Two children proved to be inoperable. Eight other cardiac extra pathologies had a specific surgery for each case. The average stay in the Netherlands was 1 month 13 days (extremes: 1 - 12 months). The evolution was favourable for 35 children all pathologies included. Four deaths occurred in the Netherlands and 2 in Congo. The organization of the technology transfer would be probably a better choice in the future.
